That's the problem with using potty pads... it trains them to pee on pads. You'll have better luck now, and less frustration switching when he's older, if you start over and consistently take him outside to potty.Outside his crate, he will pee anywhere (even in the house) if he doesn't see his pee pad. Even when he does, he will pee on anything that looks like a mat on the floor.
12 hours, that is way too long for a puppy. Pretty extreme even for an adult. Take him out more often. Like every couple of hours, and immediately after he wakes up, eats/drinks, or exercises. Better to try more often than have more setbacks. Try adding more time in between once he is consistently not having accidents.Without his bed, he can usually hold it for about 12 hours max
